There's very little you need to do to prepare for your website. The basic requirement is usually just a website hosting provider who will provide a 'home' for your website on the internet. There are many of these available, but I usually recommend Clook.net, who provide a good service.
Again, that's a question to which the answer is dependant on a great many things.
For simple sites, with a handful of pages, we'll most likely be able to start straight away, and develop on-the-fly, as it were, and see development progress from the ground up.
For the more complex builds, though, it'll be necessary to put together a project specifications document to determine exactly how long the site will take. Some sites can take many months to get to a fully working state, but that time is spent ensuring that every feature is fit for purpose, and that every attribute of the site is as well rounded as can be
A rule of thumb though - for any good website, you should probably spend as long thinking about it and planning it as you are going to build it, so I can generally give you an idea of timescale once you come to me with your site outline

Because I don't work from templates, I build sites from the ground up and therefore put plenty of consideration into the why's and who's and what's of web design.
Namely - 'Why are people going to be visiting this site?', 'Who are these people going to be?' and 'What do they expect to see on this website?'. From there I can build up a site which will maximise visitor throughput and increase efficiency.
